If a Time Lord regenerates so much, how do the people know he's the doctor when they last saw him so many faces ago?

In the case of River she's usually in his future so already knows his latest face before he does. Otherwise, with the exception of fellow Time Lords and Lady's, they don't unless his speech, possessions, or mannerisms tip them off.
